Dear O.F.A.H. Member:

We are pleased to provide the Accidental Death and Dismemberment (AD&D) benefit listed below for each O.F.A.H. member at NO COST TO YOU, provided through AIL Canada - the insurance company that specializes in providing organizations’ members with exciting and practical supplemental benefits. The Ontario Federation of Anglers and Hunters is proud to continue in partnership with AIL Canada to sponsor this benefit.

A $4,000.00 Accidental Death and Dismemberment Benefit
Provides coverage 24 hours a day, anywhere around the world. In addition, you will also receive:

* Family Eye Care Program
    Covers you and your family, providing discounts on eyewear products

NEW Child Safe Kit
    Endorsed by the International Union of Police Associations;
    Helps record important information about the children in your life

We ask you to fill out your reply card to have your AD&D certificate delivered and to designate your beneficiary.

Please remember, if you do not designate your beneficiary, the benefits will be paid to your Estate, which may be taxable.

In order to obtain the eye care program and child safe kits, the reply card must be completed and submitted to AIL Canada.
