Since the OFAH is currently participating in a group enrolment, you have recently received a letter and reply card in the mail. The reply card may also be filled out online, as indicated below. By completing and submitting the form you are requesting to receive your free optical benefits and the certificate for the accident policy, which has already been automatically set in place for you. An AIL Representative will contact you to deliver your no-cost benefits and explain any additional benefits that you and your family may qualify for once you send in the reply card.
